  2. Identify Elopement Risk Factors: List five risk factors across cognitive, medical/medication, environmental, and behavioral domains.
  3. Screen & Tier New Admissions: Screen for elopement risk on admission and assign a facility-defined risk tier.
  4. Reassess on Triggers: Describe triggers (change in condition, new meds, unit move, etc.) and initiate escalation when present.
  5. Apply Person-Centered Interventions: Implement two interventions matched to the resident’s life story and known triggers during high-risk times.
  6. Verify Environmental Safeguards: Check alarms, door status, and sightlines at start of shift; report malfunctions immediately per policy.
  7. Targeted Rounding & Headcounts: Perform focused rounds and complete headcounts at unit transitions; document frequency and findings.
  8. Activate Elopement Response Protocol: Announce code, confirm last-known location/time, cover assigned search zone, and report updates at set intervals.
  9. Safe, Compassionate Re-Approach: Use validation and redirection; ensure safe return and handoff to nurse for assessment.
  10. Document & Debrief: Record timeline, notifications, assessment, and interventions; participate in post-event debrief with at least one corrective action, assigned owner, and due date.
